This promotion is available for new patients only.
Sign-ups for our email list and text message updates will receive $75.00 off their first month of their membership.
Customer accounts will be credited only after providing the correct promotional code.
This promotion is for new patients only and can only be used once.
In order to receive the $75.00 discount, patients must be actively subscribed to the newsletter at the time of purchase.
Signing up for both our email list and text message updates will not allow patients to receive $75.00 twice.

Mothers Day Promotion
Includes 4 Semaglutide injections for just $150 (regularly $375).
The Mother’s Day promotion applies to new patients only or patients who have not had a membership with us after March 1st.
This promotion is only valid for one 4-week vial Semaglutide 0.25mg.
Promotions cannot be combined. One promotion per patient. Membership must begin before May 31st, including physician’s visit and monthly membership start up.
This promotion waives the $95 new patient physician’s visit fee and the $75 returning patient physician’s visit fee.

New Year 2025 Promotion
New members save $75 on their first month of GLP-1 medications (Semaglutide or Tirzepatide) by providing us with the correct promo code when scheduling their initial consultation.
The New Year promotion applies to new patients only or patients who have not had an active membership with us after December 1st, 2024.
This promotion is valid only for the first month of GLP-1 treatments, including Semaglutide or Tirzepatide.
Promotions cannot be combined. One promotion per patient.
Membership must begin before January 31st, 2025, including physician’s consultation and monthly membership activation.

"All Summer Long" Promotion
This promotion is available for new patients only.
Patients who use the code "ALLSUMMER" will receive compounded savings on their Tirzepatide or Semaglutide medications.
This promotion applies only to patients on Tirzepatide or Semaglutide medications.
The promotional offer applies savings in the following order after the patient has provided Hormone Fitness with the code through the form of a written notice, email, text message, or when submitted with an appointment scheduled on hormonefitness.com.
$50.00 off the first month (or the first billing cycle for active patients) after submitting the applicable promotional code.
$75.00 off the second month (or the second billing cycle for active patients) after submitting the applicable promotional code.
$100.00 off the third month (or the third billing cycle for active patients) after submitting the applicable promotional code.
This offer only applies to three consecutive months of Tirzepatide or Semaglutide treatments. If a payment is not received within 10 business days of the due date for each billing cycle the offer will expire. This also included pausing your membership for longer than 10 consecutive business days.

Second Chance Promotion
This promotion is only for patients who received an email from Hormone Fitness with a code and instructions on how to redeem it.
Patient's who received this email had scheduled, completed, or canceled a complimentary consultation and (or) a physician consultation with us in the past 5 years. Patients may have also received this email if they had previously signed up for a membership with us and canceled their membership in the past 5 years.
The promotional offer applies $95.00 off the patients first month of treatment after the patient has provided Hormone Fitness with the code through the form of a written notice, email, text message, or when submitted with an appointment scheduled on hormonefitness.com.
Each individual patient and(or) membership can apply this code once.
This promotion can not be used in combination with other active promotional codes.
This promotion waives the $95 new patient physician’s visit fee.

Returning Patient Promotion
This promotion is available for returning patients only who are not currently on a membership/billing cycle.
Patients who use the promo code provided by Hormone Fitness will receive $50 off their next monthly billing cycle when they restart their membership for Hormone Fitness.
This promotion applies only to patients on Tirzepatide or Semaglutide medications.
The promotional offer applies savings only after a patient has started their monthly billing cycle.
$50.00 off the first month (or the first billing cycle for active patients) after submitting the applicable promotional code.
If a payment is not received within 10 business days of the due date for the set billing cycle, then the offer will expire.
This also includes pausing your membership for longer than 10 consecutive business days.
Each individual patient and(or) membership can apply this code once.

$55.00 Save Promotion
This promotion is available for existing patients only.
Patients who accept the 55 SAVE Promo will receive 3-months of savings on their Tirzepatide or Semaglutide medications.
This promotion applies only to patients on Tirzepatide or Semaglutide medications.
The promotional offer applies savings to 3 consecutive months of a membership.
$55.00 off the first month (or the first billing cycle for active patients) after submitting the applicable promotional code.
$55.00 off the second month (or the second billing cycle for active patients) after submitting the applicable promotional code.
$55.00 off the third month (or the third billing cycle for active patients) after submitting the applicable promotional code.

Promotional Submissions
Virtual Interviews
This promotion is available for active patients only.
Virtual interview testimonials must be applied for, are subject to a quality standard based on Hormone Fitness needs for marketing purposes. Credit will be applied to the patient's account at managements’ discretion.
Credit applied is up to $375 off one month (30 days) of your treatment cost upon approval. If you are on our maintenance program and your treatment is $199 per month, you will be credited $199.
Video Release Form:
I, hereby grant Hormone Fitness and its representatives the right and permission to use, reproduce, and publish photographs and/or video recordings of me for marketing, promotional, and educational purposes.
I understand and agree that these images may be used in various media outlets, including but not limited to the company's website, social media platforms, brochures, newsletters, and other promotional materials.
I understand and agree to all video and audio being recorded during the virtual interview.
I release Hormone Fitness and its representatives from any claims, damages, or liability arising out of or connected with the use of these photographs and/or video recordings.
I understand this document may contain information that is health related. I agree that by signing this document, I am allowing the information to potentially be published/posted.
I understand that my name or any other personal information will not be disclosed or used in conjunction with the images without my express consent.
Patient can only redeem one rebate per billing cycle. Can’t combine rebates in one billing cycle.
Before and After Photo Submissions
- This promotion is available for
active patients only.
- As of July 1st, 2024, if you submit a Before and After pair of photos and they are approved by our management team, then you will receive $175 off your next monthly billing cycle.
- Credit applied per membership referred (once confirmed) is $175. (i.e. patient is taking 7.5mg Tirzepatide for $495/month. Their next billing cycle will be $495 - $175 = $320 payment for that month). Payments on subsequent billing cycles will return to automatic payments, unless patient requests to be on manual payments.
- Before/After pictures are subject to a quality standard based on Hormone Fitness needs for marketing purposes. Credit will be applied to the patient's account at managements’ discretion.
- Only one rebate can be redeemed per billing cycle, and rebates cannot be combined within a single billing cycle.
Photo Release Consent
- By accepting the terms and conditions when submitting before and after photos, you grant Hormone Fitness and its representatives the right and permission to use, reproduce, and publish photographs and/or video recordings of me for marketing, promotional, and educational purposes.
- By agreeing to these terms and conditions, permission is granted for Hormone Fitness and its representatives to use images in various media outlets, including but not limited to the company's website, social media platforms, brochures, newsletters, and other promotional materials.
- Hormone Fitness and its representatives are released from any claims, damages, or liability arising from or connected with the use of these photographs and/or video recordings.
- This document may contain health-related information, and by signing, consent is provided for this information to potentially be published or posted.
- By accepting the terms and conditions you give Hormone Fitness the permission to use your first name and last initial in conjunction with the before and after photos.
- All other forms of personal information will not be disclosed or used in conjunction with the images without express consent.
Photo Quality Standards
- When taking before and after weight loss photos, ensure consistent lighting, background, and pose for accurate comparison.
- In the before photo, wear minimal clothing, stand naturally, and maintain a neutral expression.
- Replicate a similar setup as shown in the quality standard checklist when submitting your photos on our website.
- This setup should showcase your transformation with the same clarity and authenticity.
- Optionally, include progress markers like measuring tape or scales for additional context.

Customer Referrals
If, as of January 8th 2024, you have shared our information with friends or family, and they have expressed interest or scheduled a consultation with us or plan to in the near future, kindly provide their details below.
This promotion is available for active patients only.
Credit applied per membership referred (once confirmed) is $175. (i.e. patient is taking 7.5mg Tirzepatide for $495/month. Their next billing cycle will be $495 - $175 = $320 payment that month).
Credit will be applied to the patients account at managements’ discretion.
All referred patients must be verified before applying credit. Verification includes: referred patient must have a membership set up, the first month paid for and their first script submitted to the pharmacy.
Any previously referred patients (before 1/8/24) are not eligible for this promotion.
